By long-term, we mean a chronic illness, disease or other condition for which there is no cure. Medication and other treatments are used to control the condition and help people live as normal a life as possible.
GP practices will invite anyone with a long-term condition to an annual review usually in their birth month. The purpose of the review is to see how your condition is affecting your health and wellbeing. If your condition has got better or worse, you can discuss the potential causes and any improvements that can be made.
Attending your yearly review is important, even if you are feeling well. This is so we can monitor your progress and review your medications etc to ensure your condition is still being managed appropriately. Yearly Long-Term condition reviews are also beneficial in reducing acute illness and therefore reduce the number of GP and A&E visits throughout the year.
