    Care Co-ordinator My name is Heather Edwards and I have transitioned into a new role within the practice as a Care Co-ordinator. Care Co-ordinators help to co-ordinate and navigate care across the health and care system, helping people make the right connections, with the right teams at the right time. I work with patients with certain Long Term Conditions who may need extra support within their healthcare, whether it be arranging appointments, attending hospital appointments, looking for extra support or general signposting. What I can help you with: • Being a central point of contact for you/your carer. • Making sure you have the correct support in place. • Helping you connect to the services/support you require, whether this be at Humshaugh & Wark Medical Group or elsewhere. • Supporting you to prepare for shared decision-making conversations with health professionals. • Helping make sure that your care is personalised and that those involved in your care understand what is important to you. • Helping you complete a personalised care and support plan if one is required. • Providing links to information to help you understand your health condition. What I can't help you with: • Any medical questions you may have. My normal working hours are Wednesday, Thursday and Friday and can be contacted during these times.

Reception/Care Navigation

All our reception and dispensing staff are here to help you. They answer the phone, deal with enquiries, carry out administration tasks, take repeat prescriptions and dispense medication. Their job is very demanding so please be patient. There may be times when you approach the Practice for help, but more appropriate help may be better provided elsewhere. You may find that our GP’s will direct you to our reception team for advice as they have been trained in care navigation and will be able to suggest appropriate or alternative organisations that are better able to help you.
